Continuum’s Professional Care Managers are health and human services specialists — experienced and certified health administrators, elder care managers, family therapists, paralegal and financial aid specialists — who help families care for older relatives, while encouraging as much independence as possible.
Carol Hatton, MHA, CMC, QDCS
President and CEO
Founder of Continuum Care Management in 1995, Carol has been a member of National Association of Professional Geriatric Care Managers and a certified care manager (CMC) since 1995. She has over twenty years of diverse experience working with the elderly and their families. She has been active in dementia care, serving as a member of the advisory committee for Early Stage Alzheimer’s Symposiums coordinated by the Alzheimer’s Association of Los Angeles. She is a member of California Coalition for Compassionate Care, and The American Society of Aging. She teaches classes in dementia care and has led caregiver support groups in the Bakersfield area.
Lois Anderson, RN, CLNC
Nurse Care Manager
Lois is a Registered Nurse and Certified Legal Nurse Consultant with seventeen years experience in monitoring clients and staff in long term homes, and maintaining legal documents related to care. She has been a clinical instructor for the LVN nursing program at Bakersfield College, and has worked for a Hospice program with case management and patient and family support. Lois keeps Continuum’s clients as healthy as is possible by communicating successfully with their physicians, monitoring their cooperation with treatment plans, and checking on them for indications of new or exacerbating illness. With Lois, good Care Management, our clients can remain in their homes with just the right amount of outside support
Charlotte Cunningham, RD
Geriatric Care Manager
Charlotte, a graduate of Cal Poly San Louis Obispo, completed an internship with The University of Cincinnati Medical Center to become a Registered Dietitian. Charlotte has been assessing and auditing dietary needs and educating individuals and caregivers on best care for their individual needs since 1986. She has been a Health Educator with Kaiser, Director of Health Education Services with Gould Medical Foundation and as consultant for Chateau of Bakersfield Day Program, and The Mentor Network. Charlotte has a wide knowledge of geriatric care and of special needs of developmentally disabled adults. She keeps our clients happy and healthy in the least restrictive environments.
Emily Hatton, LVN
Care Manager
Emily is new to our care management team, but not new to Continuum. As a Certified Medical Assistant (CMA), she has been helping out for the past few years. Since 2007, Emily has been Continuum’s Account Manager while completing her academic requirements and attending nursing school. She graduated in June of 2011, successfully completed her Care Management internship, and board exams. Emily is not new to the field of geriatrics. She has been volunteering in nursing homes since she was in seventh grade. As a certified massage therapist, she completed work in geriatric massage and worked with frail clients and persons whose physical deficits required special considerations. We welcome Emily to our Care Management Staff.
